• Primary contact information
  Please note that while others may work on the application, only the primary contact can submit the completed application
• Basic company information including physical mailing address and invoice/payment email addresses
• Tax exempt information (if applicable) and W9 form
• Prescription drug and/or prescription device licensing (if applicable)
• Important: Your application must be submitted within 30 days of its creation. After 30 days, it will automatically be deleted.
